Medical Claim Notification Form needs to be filled for each accident and illness that SCI Insurance might cover. One side is filled and signed by the project coordinator and the volunteer. The other side is filled by the doctor.

We have Third Party Liability insurance only for volunteers sent to another country. Volunteers in their own country cannot be covered by TPL, unfortunately. In the case of Third Party Liability please find attached the Notice of claim TPL document which needs to be filled in.
