Insurance Infosheet is meant for volunteers and project hosts. It has all the information they need to know about SCI Insurance.

Medical Claim Notification Form needs to be filled for each accident and illness that SCI Insurance might cover. One side is filled and signed by the project coordinator and the volunteer. The other side is filled by the doctor. Send the filled document to your organisation.
In the case of Third Party Liability claim please find attached the Notice of claim TPL document which needs to be filled in.
